Patients of all Physicians and Nurse Practitioners are asked to visit the Family Health Team to enroll/ roster with the Family Health Team. This Ministry process is required in addition to the application and medical history process recently completed for new patients.

If you have recently signed a “roster” form on a visit since June, 2007 this is sufficient. For new patients, the enrollment form is separate from the application forms completed.

Benefits of Enrollment

Patients will have access to primary health care treatment advice, 24 hours a day, seven days a week, through a combination of regular and extended hours, and a nurse-staffed Telephone Health Advisory Service (THAS). Comprehensive communication will be available to access these services over the coming weeks.

Patient Enrollment Requirements

By completing and signing the Patient Enrollment and Consent to Release Personal Health Information Form, patients commit to the following:

· To seek treatment from their family doctor/ nurse practitioner in the group first, unless they are traveling or find themselves in an emergency situation

· To allow the Ministry of Health and Long Term Care to provide their doctor with information about services they have received from family doctors outside the Family Health Team, and some preventative care services

· To not switch the doctor or the Family Health Team they are enrolled with more than twice a year
