The Tilbury District Family Health Team is funded by the MOHLTC. Donations to the Tilbury District Family Health Team are used to enhance the services and access to health care provided to the patients and residents of the community. Tilbury District Family Health Team has received charitable status and is able to issue a charitable donation receipt to you for your donation made to the Tilbury District Family Health Team. We are deeply appreciative of the many donations received to date that have assisted us to advance the programs and service provided. Planned Giving

Planned giving is a method of making a charitable contribution after some event or some period has passed. Planned giving can take on two forms:
1) A legacy (life insurance, will, trust)
2) A gift during the donor’s lifetime
