The Tilbury District Family Health Team serves a community of more than 15,000 people. Located between Windsor and Chatham along the 401 corridor, our community is situated south of Lake St. Clair and north of Lake Erie and includes towns and villages such as Tilbury, Lighthouse Cove, Comber, Pointe-aux-Roches, St. Joachim, Staples, Merlin, Jeanettes Creek, Port Alma, Prairie Siding and Wheatley.
